While driving at highway speed, the factory-installed serpentine belt on my 2023 Chevrolet Express 3500 (6.6L V8, approximately 24,000 miles, purchased new, factory-maintained per schedule) failed suddenly and without warning. The belt on this engine drives the water pump and other critical accessories, so the failure caused immediate loss of engine cooling and rapid, severe overheating while the vehicle was in motion in traffic. Vehicle broke down with a crank no start condition. Verified no fuel pressure. Diagnostics revealed no continuity between tank fuel pump positive lead at fuel pump relay, and ground. Removed fuel tank, disconnected fuel pump wiring harness, and found burned pin at positive terminal of fuel pump and mating harness connector. Harness displayed significant distortion and charring consistent with a poor connection.

The OTA had the interstate lanes bottle necked down. Traffic was trying to merge. The van had already slowed down and moved into the thru lane closest to the median. The van was pushed off the road by a semi into the contrete barrier. The semi clipped the bumper on the front passenger's side. It was also hit on the driver's side rear bumper. ABS Valve Body / Pump Harness chafed through brake hydraulic line near transmission crossmember. This resulted in a hydraulic leak and loss of brake pedal feel / partial loss of brakes. Fortunately failure occurred in the driveway--I had 4500 Lbs of sailboat trailer hitched to the van and was about to go on a long trip. Design issue. Wiring harness should not be located so as to chafe on hydraulic lines. This is not the first chafe related failure on this vehicle.

I was driving approximately 70 mph on the highway when I came up behind a vehicle whose three brake lights were completely inoperative. The vehicle suddenly stopped, but there was no brake light illumination to warn following traffic. The only reason I realized the vehicle was stopping was because I was traveling on a slope and could see the vehicles ahead of it also slowing down. If the roadway had been level, I likely would not have known the vehicle was braking and could have rear-ended it…
